Thank you for choosing to have your knee, foot, ankle, shoulder or other athletic injury treated by Austin Knee Specialist Dr. Brotzman or Austin Shoulder Specialist, Dr. Graham. The sports medicine services provided by our Austin sports injury surgeons are restricted to the portion of medicine referred to as orthopaedic surgery, which is the practice of caring for bones, joints, and the muscular components of the body. An orthopaedic surgeon is a physician who has received extensive additional specialized training for the treatment of these parts of the body, particularly when surgical correction is required. Both Dr. Brotzman and Dr. Graham are certified by the American Board of Orthopaedic Surgery.

Whether you are coming in for knee surgery or treatment, foot injury rehab or treatment, ankle injury diagnosis or treatment, shoulder injury or treatemant treatment or any other sports related injury, you will need to fill out the below forms before seeing Dr. Brotzman or Dr. Graham. In order to expedite the New Patient registration process and to help minimize your waiting time, we have provided our patient information forms below for you to fill out prior to your visit. This is required, so doing this prior to your visit will save you 30 to 40 minutes.

INSURANCE GUIDELINES FOR AUSTIN SPORTS INJURY TREATMENT:
Our specialized sports injury physicians are contract providers for most insurance plans. If we are a provider for your insurance company, we will bill your insurance company for payment. However, you will be expected to pay your co-payment, co-insurance, deductible amounts, and/or non-covered items at the time of each visit.
